Responsibilities
- Perform initial evaluations and develop individualized PT plans of care for residents in a skilled nursing setting.
- Provide therapeutic interventions including therapeutic exercise, gait training, balance activities, manual therapy, and functional mobility training.
- Monitor progress, re-evaluate as needed, and modify treatment plans to achieve stated goals.
- Educate residents and families on treatment plans, home exercises, and safety strategies; coordinate with the interdisciplinary team.
- Maintain accurate, timely documentation of evaluations, progress notes, treatment plans, and discharge summaries in accordance with facility and regulatory guidelines.
- Ensure patient safety during transfers and activity, and adhere to infection control, safety, and privacy policies.

Requirements
- Current Physical Therapist license to practice in the United States (state-specific licensure requirements to be confirmed).
- Ability to work 40 hours per week in a long-term care setting with a consistent 08:00-16:30 schedule.
- Availability to start by 08/31/2026 and commit to a 13-week assignment.
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team.
- Demonstrated clinical competency in adult and geriatric physical therapy within a skilled nursing facility environment.
