8:00 AM - 4:30 PM with rotating weekend shifts. Job Description Selected candidates will complete their orientation and then transition to their assigned unit. This position serves in a Level II trauma hospital with a focus on high acuity and fast-paced environments. The RN Case Manager will work as part of a triad model with the RN/SW/CMA team, ensuring effective patient management and care coordination. Responsibilities Conduct initial assessments and discharge planning. Complete Interqual assessments for initial and discharge reviews. Collaborate with the multidisciplinary team to ensure optimal patient care. Manage caseloads effectively in a fast-paced environment. Required Experience / Certifications / Licensure Current RN license in Florida or Compact state. 2-4 years of RN case management experience in an acute care facility. Preferably experience with HCA facilities. Familiarity with Meditech, Midas, Nate, and Outlook. Dress Code Business casual attire with a white lab coat.
