Work Environment

This is a full-time, onsite position primarily conducted in the homes of our patients in Yuba City. You will have the opportunity to work Monday through Friday from 9 AM to 5 PM, allowing for a balanced and predictable work-life schedule. Our home care setting means you will witness firsthand the positive impact of therapy in the comfort of each patient's living environment, enhancing their engagement and comfort during rehabilitation.

Key Responsibilities

• Conduct thorough initial assessments of each patient's medical history, current physical condition, functional limitations, and personal goals to create tailored treatment plans.
• Design comprehensive and individualized plans of care that prioritize patient engagement and understanding, ensuring they are actively involved in their rehabilitation process.
• Implement effective treatment strategies that include manual therapy techniques, progressive exercise prescriptions, and patient education to facilitate recovery and improve mobility.
• Regularly monitor and document patient progress, adapting treatment approaches based on outcomes and patient feedback to optimize care.
• Maintain accurate and thorough documentation of all services and care provided, adhering to legal and organizational standards.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic conditions, injuries, and surgeries. You will work closely with individuals seeking to regain mobility and strength, as well as those aiming to manage pain or improve their physical resilience. Understanding the unique challenges each patient faces is vital in crafting personalized and effective care plans that address their specific circumstances.

Qualifications

• A Doctoral Degree in Physical Therapy (DPT) from an accredited academic institution.
• Active licensure as a Physical Therapist in the state of California.
• A clean professional record with no disciplinary actions or board actions against your license.
• A minimum of three years of demonstrated experience in orthopedic physical rehabilitation.
• Strong skills in manual therapy techniques and experience in developing and supervising exercise regimens.

Preferred Experience

• Experience working in a home care environment, adapting treatment strategies to suit individual patient settings.
• Familiarity with electronic health record (EHR) systems for efficient documentation and patient tracking.
• Certification in advanced manual therapy techniques or specialized orthopedic rehabilitation programs is a plus.

Compensation and Benefits

This full-time role offers a competitive salary range of $90,000 to $110,000 annually, commensurate with experience.
