Pay: Approximately $27/hour + Benefits

About the Position

We are a growing HCBS program looking for a warm, organized, detail-oriented Service Coordinator to join our Hempstead team.

This is a great opportunity for someone who enjoys working with families, solving problems, navigating services and insurance requirements, and making sure participants receive the services they need.

Responsibilities

• Manage an HCBS participant caseload

• Coordinate services, schedules, and DSP staffing

• Work with families, providers, care managers, and insurance/Medicaid Managed Care Plans

• Obtain and track service authorizations and renewals

• Monitor service utilization and address gaps in services

• Maintain accurate and timely documentation

• Troubleshoot authorization, scheduling, billing, and service-delivery issues

• Help participants and families access needed resources and supports

• Ensure compliance with HCBS and Medicaid requirements

Who We're Looking For

• Kind, positive, dependable, and highly detail-oriented

• Strong problem-solving skills and a "get it done" attitude

• Comfortable communicating with families and navigating paperwork

• Experience with HCBS, government programs, insurance, healthcare, social services, psychology, or case management is a plus

• Bilingual Spanish is a plus

• Strong organizational and computer skills

• Able to work independently and as part of a supportive team
