Home Health Registered Nurse

Schedule

This position requires availability from Monday to Friday, 8 AM to 5 PM, with some weekend coverage. Additionally, some call may be scheduled, but only on an emergency basis.

Job Description

As a Home Health Registered Nurse, you will provide high-quality patient care in home settings. Your responsibilities will include conducting assessments, creating care plans, and delivering individualized nursing care to patients. You will be expected to manage changes in your schedule effectively and prioritize patient needs.

Responsibilities

• Perform OASIS assessments for home health patients.
• Develop and implement personalized care plans.
• Travel to patients' homes to provide nursing care.
• Collaborate with healthcare professionals to ensure quality care.
• Maintain accurate and up-to-date medical records using EMR system HealthWyse (Careficient).

Required Experience / Certifications

• Must have OASIS experience.
• Home health and hospice experience is required.
• Experience in managing a traveling schedule for patient visits.
• Must hold a valid driver's license in good standing.
• Basic Life Support (BLS) certification required.
