Key Responsibilities\n

• Facilitate connections with community resources to coordinate post-discharge services and ensure continuity of care.

• Provide crisis intervention and emotional support, assisting patients and families during moments of acute distress.

• Document patient interactions and care planning accurately and timely, ensuring compliance with hospital policies and legal standards.

Qualifications\n

Compensation and Benefits\n

The compensation range for this position is between USD 70,000 and 100,000 annually, commensurate with experience and qualifications.
