We are currently seeking an Advanced Practice Provider, serving local skilled nursing facilities within a close-knit, mission-driven care community. Relocation assistance is available to eligible candidates Unmatched Work Schedule Flexibility: 6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.) Freedom from on-call and overnight requirements Timely Charting using a company-provided iPad with LTE Technology that Makes Work Easier Document efficiently using an EHR designed to streamline workflows Voice dictation [and AI Scribe] for fast and accurate documentation Access to UpToDate® for instant clinical guidance What You’ll Do Provide high-quality, patient-centered care to residents in skilled nursing facilities Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues Communicate with facility nursing staff via phone or secure messaging as needed Maintain reliable on-site attendance for scheduled shifts Ensure compliance with clinical standards, facility policies, and regulatory requirements Your Qualifications Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
