About the Role:
Join Bellestar Home Health Care Inc as a Home Health RN and make a difference in the lives of patients in Amador, CA. We are looking for compassionate and dedicated registered nurses to provide exceptional care in a home setting, ensuring comfort and support for our clients.

Responsibilities:

• Conduct comprehensive assessments of patients' health and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' progress and adjust care plans as necessary.
• Educate patients and their families on health management and disease prevention.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Document all patient interactions and care provided in compliance with regulations.
• Provide emotional support and counseling to patients and families.
• Participate in ongoing training and professional development opportunities.

Requirements:

• Active Registered Nurse (RN) license in California.
• Minimum of 1 year of nursing experience, preferably in home health or community settings.
• Strong clinical skills and knowledge of patient care practices.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate demeanor with a focus on patient-centered care.
• Basic Life Support (BLS) certification required.
• Familiarity with electronic health records (EHR) systems is a plus.
