1 day ago Be among the first 25 applicants Overview Job Overview – Care Manager (Children’s Health Home): Compensation: $26/hour Location: Brooklyn, NY Schedule: Monday to Friday, 8:00 AM to 5:00 PM Our client is hiring a Care Manager in Brooklyn, NY, to support children and youth with complex medical, behavioral, and developmental needs. This field-based role involves conducting assessments, developing care plans, and coordinating services across providers, families, and community resources. Bilingual Spanish/English proficiency is strongly preferred, and familiarity with the CNNX system is a plus. Responsibilities Enrollment & Documentation: Support families through enrollment and complete required forms, including consents, demographics, and eligibility verification. Assessment & Care Planning: Conduct CANS-NY assessments and health screenings, and develop individualized service plans based on child and family needs. Care Coordination: Collaborate with families, case planners, and providers to align services across medical, behavioral, and social systems. Ongoing Monitoring & Transitions: Maintain regular contact with clients, monitor changes in condition, and manage transitions between care settings. Compliance & Follow-Up: Ensure timely documentation, billing, and follow-up care using systems like CNNX, while supporting treatment adherence. Qualifications Education: Bachelor’s degree in Health Services, Social Work, Psychology, or a related human services field is required. Experience: 1+ years of experience working with children and youth, particularly those with mental health, developmental, or physical disabilities. Technical Skills: Familiarity with the New York State CNNX (Connections) system is preferred, with the ability to learn and use care management platforms upon training. Skills & Attributes: Excellent communication, time management, and documentation skills, with a collaborative and client-centered approach. Licensure: Must possess a valid New York State ID.
