New Alternatives for Children, Inc. (NAC) is an award‑winning not‑for‑profit agency in Midtown Manhattan dedicated to serving children and families with medical complexity, chronic health conditions, significant disabilities, and behavioral challenges. To assist in achieving this mission, NAC is currently seeking a Bilingual Care Manager to join its Champions for Children's Health Department. Job Details Title of Position: Bilingual Care Manager Position Type: Full Time Pay Range: $61,000 – $66,000 (additional pay for licenses) Location: 825 7th Ave, New York, NY 10019 Department: Champions for Children's Health Description of Responsibilities Works closely with the child, their caregiver(s), the child's care team at NAC, and community providers. Secures required program enrollment consents from the caregiver/consenter. Completes initial and ongoing needs assessments (CANS‑NY) and enters data into the NYS online portal. Creates and manages the child’s Individualized Plan of Care (POC), coordinating service provision according to acuity level. Coordinates provision of services, supports adherence to treatment recommendations, monitors and evaluates child’s needs including wellness care, medical and mental health conditions, transitions, and community services. Meets client contact requirements, including scheduled home visits based on acuity. Completes Care Management documentation promptly using the Care Management Portal (MAPP) and EHRs. Advocates for clients within the agency and with external providers. Promotes prevention and wellness care, linking enrollees with resources and services in line with their needs. Educates child/caregiver(s) on care of chronic conditions, immunizations, screening and preventive interventions. Helps clients obtain Medicaid‑covered or managed‑care plan services. Communicates effectively with clients, considering language, literacy and cultural preferences. Coordinates care planning meetings/conferences, serving as an interdisciplinary team member. Identifies community resources and actively manages referrals, access, engagement, follow‑up and coordination. Participates in discharge planning during hospital admissions to ensure smooth transition home. Attends ongoing staff development trainings to enhance relevant skills. Ensures child receives periodic evaluations and follow‑up treatment for dental, vision and hearing care per Medicaid EPSDT guidelines. Performs additional duties as required by agency. Required Education & Experience Bilingual (Spanish/English). A Master’s Degree in Social Work, Health Education, Community Health or a related field. Prior experience in Bridges to Health, Care at Home, or Targeted Case Management programs is a plus. Required Skills Extensive experience serving children and families in child welfare, developmental disabilities, mental health, health care, or related systems. Experience in service coordination, information sharing, linkages, and referrals for community‑based social and health‑care services. Required Competencies Excellent communication and organizational skills. Ability to work collaboratively with an interdisciplinary care team. Strong initiative and interpersonal ability to negotiate/advocate on behalf of clients. Passion for serving children and families with special needs. Willingness to travel to community agencies, participant communities, and homes within the catchment area. Proven self‑management skills, meeting deadlines, ensuring compliance with agency policy and maintaining records per contractual requirements.
