Nurse Practitioner for In-home Visits

Position We’re hiring board‑certified nurse practitioner contractors to conduct in‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on a person’s overall health and well‑being. Identify potential barriers to care. Perform medication and medical history review and a physical evaluation. Use a pre‑populated, patient‑specific form for each evaluation. Do not prescribe medicine, order lab tests, or alter existing treatment regimens. Compensation Earn up to $2,500–$4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment). Increase per‑visit rate up to 135% or 160% depending on working counties with Market Multiplier. Earn additional compensation via pay‑per‑test model. Availability incentives based on weekly availability. Schedule and Travel Shifts available Monday‑Sunday (no nights/weekends required). Short‑term or Long‑term basis. Airfare, hotel, and rental car available depending on eligibility. Other Details Independent Contractor (1099) status. Malpractice coverage. State license required. Flexible schedule.
