Registered Nurse Care Coordinator - Life Care Center of Altamonte Springs Location: Altamonte Springs, FL Program: Institutional Special Needs Plan (ISNP) Schedule: Monday-Friday, 8:00 AM-5:00 PM Benefits: Weekly pay, Medical, Dental, Vision Pay: $40/hourly Position Overview We are seeking an experienced and compassionate Registered Nurse (RN) Care Coordinator to support our ISNP Program at Life Care Center of Altamonte Springs . This is a temporary assignment covering an FMLA leave, offering the opportunity to work closely with advanced practice clinicians, facility staff, residents, and interdisciplinary care teams to coordinate high-quality patient care. The RN Care Coordinator will play a key role in ensuring comprehensive care management, maintaining accurate documentation, supporting quality initiatives, and assisting with clinical coordination for a medically complex geriatric population. Key Responsibilities Ensure accurate, thorough, and compliant clinical documentation. Collaborate with interdisciplinary care teams to support patient-centered care. Assist providers and care teams with care coordination activities. Support benefit determination related to Medicare Part A, Part B, and Part D services. Partner with the Advanced Practice Clinician (APC) and facility staff to identify and respond to changes in patient condition. Coordinate therapy services and follow-up care needs. Participate in facility-based meetings and care planning discussions. Schedule and participate in family conferences, team meetings, and case presentations. Assist with quality initiatives, compliance tracking, and performance improvement activities. Develop and facilitate educational in-services for facility staff and care teams. Support providers with documentation, coding accuracy, medication reconciliation, assessments, nursing rounds, and clinical chart reviews. Document and communicate verbal orders from providers in accordance with state nursing regulations and facility policies. Help manage members experiencing acute medical conditions or exacerbations of chronic illnesses, including coordinating diagnostics, testing, and interventions. Support members through short-term rehabilitation and transitional care episodes. Required Qualifications Active, unrestricted Registered Nurse (RN) license in the state of Florida or ability to obtain licensure prior to start date. Associate Degree in Nursing (ADN) or higher. Current Basic Life Support (BLS) certification. Ability to perform physical assessments and lift up to 30 pounds. Reliable transportation. Ability to travel locally between facilities as needed. Full COVID-19 vaccination (subject to federal, state, local, client, and employer requirements). Ability to pass a pre-employment drug screening. Preferred Qualifications Minimum of 1 year of clinical nursing experience, preferably in a long-term care, skilled nursing, or post-acute setting. Experience working with medically complex patients, including behavioral, social, and functional health needs. Strong understanding of geriatrics, chronic disease management, and care coordination. Proficiency with electronic medical records (EMR) and clinical documentation systems. Exceptional communication, organizational, and time-management skills. Ability to work independently while collaborating effectively with multidisciplinary care teams. Comfortable working in diverse healthcare environments and managing multiple priorities.
