Job Description Case Manager RN – Full-Time (Permanent) Location: Tallahassee, FL 32310 Schedule: Day Shift (8:00 AM – 4:30 PM) 1 Weekend every 4 weeks Compensation: $31.94/hr – $43.92/hr (based on experience) + $10,000 Sign-On Bonus + Relocation Assistance Key Responsibilities Conduct comprehensive clinical & psychosocial assessments Develop and implement individualized care plans Coordinate with interdisciplinary teams Perform utilization reviews & ensure medical necessity Lead discharge planning & remove care barriers Collaborate with physicians, families & external agencies Ensure compliance with hospital & regulatory standards Qualifications Education: ADN required | BSN preferred Experience: 3+ years Case Management experience in acute care OR 3+ years in Med/Surg, ICU, ED, Tele, PCU, or Neuro (Home Health/Insurance background welcome with strong acute care experience) Licensure: Active Florida RN License (or Compact; FL required within 90 days) Compensation Details $31.94/hr – $43.92/hr (based on experience) Evening Diff: +$2.50/hr Weekend Diff: +$2.00/hr $10,000 Sign-On Bonus (paid over 2 years) Relocation Assistance Available Benefits Medical, Dental, Vision Coverage 401(k) with 100% match (3%–9%) Tuition Assistance & Loan Support Free Telemedicine & AirMed Transport PTO, Family Leave & Wellness Programs Pet Insurance, Legal Aid & More Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
