Bilingual RN Care Manager

Responsibilities Identify and resolve barriers that hinder effective care. Ensure patients are progressing toward desired outcomes by managing care through assessments and evaluations. Monitor member progress and adjust care strategies to achieve optimal outcomes. Assist in developing and implementing individualized care plans. Collaborate with interdisciplinary teams to coordinate care and services. Manage daily tasks, maintain accurate documentation, and follow established guidelines and procedures. Use clinical knowledge to assess and evaluate member needs to maintain optimal wellness. Communicate and engage with members through telephonic care management. Required Qualifications Bachelor’s Degree in Nursing (BSN). Bilingual in English and Spanish with ability to pass language proficiency tests in both languages. Active RN license without restrictions in Puerto Rico. Active RN license without restrictions in Florida. Affiliation with the CPEPR (Colegio de Profesionales de Enfermería de Puerto Rico). Prior clinical experience in adult acute care, skilled nursing, rehabilitation, or discharge planning. Knowledge in chronic condition management (Diabetes, Hypertension, COPD, chronic kidney disease). Ability to work an 8‑hour shift between 8:30AM and 5:30PM EST (adjusted for Daylight Savings). Preferred Qualifications Health plan experience. Previous case management experience. Call center or triage experience. At least 1year of experience managing Medicare members. Work‑At‑Home Requirements Minimum download speed of 25Mbps and upload speed of 10Mbps; wireless, wired cable or DSL recommended. Satellite, cellular or microwave connections are allowed only with leadership approval. Work from a dedicated space with no interruptions to protect member PHI/HIPAA information. Humana will provide telephone equipment required for the position. Language Proficiency Testing All associates speaking a language other than Spanish must take the Interagency Language Rating (ILR) test. Travel Occasional travel to Humana’s offices for training or meetings may be required. Schedule 40 hours per week, 8‑hour shift between 8:30AM–5:30PM EST.
