Job Summary:
Walnut Creek Nursing and Rehab is seeking a motivated and compassionate Full-Time Physical Therapist (PT) to join our interdisciplinary healthcare team. As a vital member of our rehabilitation department, you will work closely with our experienced therapists, nurses, and physicians to provide high-quality physical therapy services to our residents. Responsibilities:

• Conduct thorough evaluations and develop individualized treatment plans for residents with a wide range of medical conditions and injuries

• Implement and modify physical therapy interventions to promote optimal mobility, strength, flexibility, and function

• Provide direct care to residents, including one-on-one therapy sessions, group therapy, and education to residents, families, and caregivers

• Document all therapy sessions and progress accurately and in a timely manner

• Collaborate with other healthcare professionals to ensure comprehensive care and coordinate efforts to achieve resident goals

• Participate in care conferences and interdisciplinary team meetings to discuss resident progress and develop care plans

Requirements:

• Doctorate in Physical Therapy (DPT) or Bachelor's degree in Physical Therapy (PT)

• Current Pennsylvania state licensure as a Physical Therapist

• Ability to work in a fast-paced environment and adapt to changing priorities

• Excellent communication and interpersonal skills
