As a therapist at CenterWell Home Health, reporting to the Home Health Branch Administration, you will help patients regain strength, mobility, and independence—all from the comfort of their homes. Responsibilities Assess and screen patients’ daily living and work‑related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist physicians in evaluating patient function. Confer with the patient’s physician and other healthcare team members and participate in the development and revision of the Plan of Care Treatment. Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range of motion, and function, or to teach compensation measures. Review and expand retraining programs in a manner consistent with each patient’s behavioral goals and the overall rehabilitation program guidelines. Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans consistent with patients’ needs and capabilities. Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members. Participate in care coordination and discharge planning activities and act as a resource to other healthcare personnel in meeting patient needs. Design community reintegration activities to assist the client in physical reconditioning and/or psychological adjustment and coordinate the plan with the interdisciplinary team. Recommend and/or design special adaptive equipment to improve the patient’s residence or working environment and to enhance participation in the rehabilitation program. Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings. Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services. Accurately and thoroughly document patients’ care observations, interventions, and evaluation on the day services are rendered; ensure that interim verbal orders from the physician are accurately documented and implemented. Submit evaluation, treatment plans, and discharge summaries to the supervisor and care management staff. Valid driver’s license, auto insurance, and reliable transportation. Preferred Qualifications Minimum six months of occupational therapy experience. Home Health experience. Pay Range USD 79.00 - 109.00 per visit/unit; USD 115,300 - 158,600 per year base pay. Additional Information TB Statement This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB. Driving Statement This role is part of Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance. The insurance must meet the state’s minimum required limits or $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 property damage, or the higher amount, whichever is greater. Scheduled Weekly Hours Full‑time (40 hours per week). The compensation range may vary based on geographic location, skills, education, and experience.
