Southwoods Health is seeking a compassionate and skilled Per Diem Speech Therapist to join our growing Home Health team. In this role, you won't just provide therapy-you will help patients regain their independence and quality of life in the comfort of their own homes.

Your Impact as a Home Health SLP:

• Patient-Centered Care: Conduct comprehensive evaluations and administer medically prescribed treatments to help patients overcome communication, cognitive, or swallowing disorders.
• Clinical Collaboration: Partner with physicians and multidisciplinary teams to develop and adapt treatment plans based on patient progress and medical necessity.
• Family Education: Empower patients and their families by providing essential education on recovery processes, physician protocols, and home exercise programs.
• Documentation Excellence: Maintain accurate clinical records in accordance with state, federal, and Southwoods standards, ensuring timely reporting of patient outcomes.
• Mentorship: Provide supervision for students and support personnel in alignment with Ohio licensure requirements.

What You'll Need to Succeed:

• Education: Graduate of an accredited Speech-Language Pathology program.
• Licensure: Active Ohio State Licensure as a Speech Therapist.
• Certification: Current American Heart Association (AHA) Basic Life Support (BLS) certification.
• Experience: Prior experience in Home Health therapy is highly preferred.
• Key Skills: Strong interpersonal communication, clinical problem-solving, and the ability to work independently in a home-care setting.
