Outpatient Physical Therapist

Description Westminster Canterbury of Lynchburg, a 5‑Star rated Continuing Care Retirement Community in Central VA is seeking a Physical Therapist to join our Outpatient Physical Therapy team to join our team! Full‑time position available with complete benefits package including Medical, Dental, Vision, Paid Time Off, 403B with company match, and more. Hours typically 8 AM to 4:30 PM but may require some rotating weekends. PT shall screen, evaluate, re‑evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician. Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual. Re‑assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated. Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated. Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment. Identify needs for program development and implement programs according to company policies and procedures. Complete all required documentation according to guidelines specified in Documentation Manual. Coordinate in‑services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents. Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month. Requirements Must have a degree in Physical Therapy from an accredited program. Active state license required. Excellent organizational and time management skills.
