Schedule

Assignment Details
Patient load: May visit up to 6 patients per day

Requirements
- Minimum 1 year of skilled home health experience required
- OASIS experience required
- BCLS certification required

Roles & Responsibilities
- Provide physical therapy assessments and interventions for adult and geriatric patients in a home health setting.
- Develop and implement individualized treatment plans based on patient evaluations.
- Educate patients and their families on home exercise programs and safety measures.
- Utilize various therapeutic modalities to enhance patient recovery, including gait training and mobility assistance.
- Monitor and document patient progress, adjusting care plans as necessary.

Compliance / Notes
- Client has a stated expectation that travelers may be required to float between facilities within the state.
- Time-off requests must be confirmed with the hiring manager.
- Dress code is business casual; scrubs allowed but not required.
- Free parking available.
