Case Manager, Registered Nurse

In this role, you will conduct in-home assessments, develop holistic care plans, and collaborate with providers, pharmacies, and payers to improve outcomes and close care gaps. You’ll educate patients and caregivers, address social determinants of health, and ensure appropriate use of services.
