Salary Range: $37.00-$43.00 per hour base on experience

Work Modality: In-office, Monday through Friday from 8:30am-5:00pm with potential hybrid schedule and flexible hours.

If yes, here’s how you will contribute to the team:

As a Full-Time LVN Clinical Coordinator , you will acts as a liaison between patients, physicians, and clinical staff to ensure seamless, high-quality care for our home health patients from pre-admission through post-admission. You will play a pivotal role in ensuring patients and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.

Your key responsibilities include:

• Managing care for patient throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinating services to ensure that patients' needs are met in a timely and effective manner.
• Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on patient needs.
• Monitoring the progress of patients and ensuring appropriate interventions are implemented when necessary.
• Facilitating smooth transitions of care, ensuring all services and resources are in place for each patient.
• Oversee patient care coordination, admissions and referrals, clinical documentation and review.
• Order Management and administrative oversight.
• Advocating for patients and families, ensuring their needs are met, and providing education as needed.

Must Have:

• Successfully graduated from an approved and certified LVN program
• Current and unencumbered license to practice as an LVN in the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice to Have:

• 2 years of experience as an LVN/LPN
• Active patient or bedside care experience within the past three years
