Travel Physical Therapist – Home Health-Clearlake, CA

Location: Clearlake, CA

Setting: Home Health

Assignment Length: 13 weeks, with option to extend

Shift: 8-hour shift

Schedule: Day shift; 5 x 8s; no weekends

Travel Pay: $2,582.60 - $2,869.56 weekly

Job Summary

Provide skilled physical therapy services to patients in their homes, including evaluation, treatment planning, therapeutic interventions, patient and caregiver education, and documentation. Work independently while collaborating with the interdisciplinary care team to support safe mobility and functional independence.

Job Qualifications

· Graduate of an accredited Physical Therapy program.

· Current California Physical Therapist license required.

· Current BLS/CPR certification as required by the assignment.

· Recent physical therapy experience appropriate to the assigned setting.

· Strong assessment, treatment planning, documentation, communication, and patient-care skills.

· Ability to work the listed shift and schedule.

· Home health physical therapy experience preferred.

· Valid driver's license and ability to travel between patient locations as required.

Key Responsibilities

· Perform physical therapy evaluations and establish individualized treatment plans and functional goals.

· Provide therapeutic exercise, mobility training, functional activities, and other appropriate interventions.

· Monitor patient response to treatment and modify the plan of care as needed.

· Educate patients, families, and caregivers regarding exercises, safety, mobility, and continued care.

· Complete accurate and timely clinical documentation and collaborate with the interdisciplinary care team.
