Pediatric RN Case Manager

We are seeking an experienced Pediatric RN Case Manager to support care coordination and discharge planning for medically complex pediatric patients. The ideal candidate will have a strong background in pediatric case management and the ability to collaborate across interdisciplinary teams to ensure safe and effective transitions of care.

Responsibilities

• Manage caseload of pediatric patients requiring complex care coordination
• Lead discharge planning for medically complex pediatric cases
• Collaborate with physicians, social workers, and interdisciplinary teams
• Coordinate post-discharge services and resources
• Ensure compliance with hospital policies and regulatory standards
• Provide patient and family education related to care plans and transitions

Qualifications

• Active Massachusetts RN license (required)
• Bachelor's degree in Nursing (BSN) required
• Minimum 3–5 years of nursing experience
• Prior pediatric case management experience required
• Demonstrated experience with discharge planning for medically complex pediatric patients
• Case Management certification (CCM or equivalent) preferred

Floating Requirements

• Float required
• May float to outpatient clients as needed; however, will remain within the department

Additional Notes

• Strong communication and care coordination skills are essential
• Flexibility to support department needs, including occasional weekend coverage
