MDS Coordinator - LVN

The Springs Post-Acute is a 99-bed Skilled Nursing Facility with a Post-Acute Rehabilitation setting. Our exceptional care ranges from Wound Care to Occupational, Speech and Physical Therapy. Our leadership is very supportive with a welcoming environment.

Responsibilities :

• Monitor and assess the quality of care provided to patients.
• Develop and implement plans of care for patients in accordance with physician orders and facility policies.
• Assist with the development of long-term care plans for geriatric patients.
• Coordinate patient care services between multiple disciplines.
• Educate patients and families on the importance of preventive health measures.
• Provide support to physicians, nurses, and other healthcare professionals in providing patient care.
• Maintain accurate patient records.

QUALIFICATIONS

1. EDUCATION: Nursing degree from accredited college
2. EXPERIENCE: 2-4 years of MDS Coordinator Experience is REQUIRED
3. LICENSES/CERTIFICATIONS:

• Active RN license in state of California is REQUIRED
• BLS and CPR Certification REQUIRED
