Speech Therapist, Home Visits

Become a part of our caring community As a part‑time Speech Language Pathologist at CenterWell Home Health, you will report to the Clinical Manager and support patients’ rehabilitation and functional improvement from their homes. The role requires 20–24 hours per week and is benefit eligible. Responsibilities Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of treatment and care plans. Use professional judgment in assessing and treating disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize their effects. Administer and interpret diagnostic tests and apply therapeutic treatments, including audio logic screening. Observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician. Provide instruction and training on alternative communication systems as appropriate. Offer counseling and instruction to patients, families, and healthcare staff. Maintain and submit documentation required by the company or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports. Participate in care coordination activities and discharge planning. Maintain the highest standards of professional conduct regarding confidential information, sharing information only when authorized and in the best interests of the patient. Qualifications Required Minimum six months of experience as a speech therapist / speech language pathologist. Current and unrestricted license. Current CPR certification. Valid driver's license, auto insurance, and reliable transportation. Preferred Home health experience. Pay Range $54.00 – $76.00 pay per visit/unit. $85,400 – $117,500 per year base pay. Additional Information TB Statement – This role is patient facing and is part of Humana’s Tuberculosis screening program. TB screening is required if selected. Driving Statement – This role is part of Humana’s driver safety program. A valid state driver’s license and personal vehicle liability insurance are required. Weekly Hours 24 hours per week.
