Job Description

New York Medical & Diagnostic Center (NYMDC) is seeking a dedicated, motivated, and experienced New York State Licensed Physical Therapist to join our growing multidisciplinary medical practice.

We are looking for a compassionate professional who is committed to delivering high-quality patient care, improving patient mobility, reducing pain, and helping patients achieve their rehabilitation goals.

Our medical center offers a collaborative environment where physical therapists work alongside providers from multiple medical specialties to deliver comprehensive, coordinated patient care.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, neuromuscular re-education, and other appropriate rehabilitation treatments.
• Evaluate patients with musculoskeletal injuries, chronic pain, orthopedic conditions, and mobility limitations.
• Monitor patient progress and adjust treatment plans as clinically indicated.
• Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR).
• Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care.
• Educate patients on home exercise programs, injury prevention, and rehabilitation techniques.
• Ensure compliance with clinical standards, documentation requirements, and applicable regulations.
• Maintain a professional, organized, and patient-centered treatment environment.

Qualifications

• Active New York State Physical Therapist License (Required).
• Previous experience in an outpatient physical therapy setting preferred.
• Strong clinical assessment, treatment planning, and documentation skills.
• Efficient with time management.
• Excellent communication and interpersonal skills.
• Ability to manage a diverse patient population in a busy outpatient medical practice.
• Strong organizational skills and ability to work independently and collaboratively.
