Care Manager – RN – Inpatient Medicine Units – Allen Hospital (Day Shift) Across NewYork‑Presbyterian/Allen Hospital, Care Managers are guardians of continuity of care; they are skilled clinicians who lead the full spectrum of patient care—from pre‑admission to discharge—while ensuring quality outcomes. Responsibilities As a Care Manager at NewYork‑Presbyterian, Allen Hospital, you will serve as a highly valued leader from day one. You will provide intensive care management for adult patients with complex medical needs, acting as a liaison among the interdisciplinary team of social workers, physicians, physical therapists, occupational therapists, cardiologists, and intensivists. You will direct all aspects of inpatient care, develop individualized care plans, ensure optimal length of stay, help prevent hospital readmission, anticipate outpatient service needs, and set appointments and consultations prior to discharge. Preferred Criteria 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting 1 year of leadership experience in a healthcare setting Master's Degree in a healthcare‑related field Fluency in Spanish Required Criteria Bachelor's degree in Nursing (BSN) Current NYS Registered Nurse (RN) licensure 3 to 5 years of health care experience in an acute care setting Superior communication, problem‑solving, business acumen, and interpersonal skills Other Requirements The position is day shift, Monday‑Friday with rotating weekend/holiday coverage. All roles require on‑site presence; therefore, employees should live within a commutable distance to NYP. NYP will not reimburse travel expenses. Salary Range & Benefits Annual salary range: $134,900–$168,000.
