Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and departmental policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include the coordination, ensuring, and providing of direct patient care activities on the unit. This involves the provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is vital. Providing patient and family-centered education is a key component of care. Care coordination and individualized treatment planning are also essential. The RN conducts comprehensive assessments in accordance with the nursing scope of practice and hospital policies/procedures. The RN considers the patient's culture, age, and developmental level during assessments. Identifying patient risk factors and presenting complaints is crucial. Obtaining patient health information from family or others as necessary is part of the role. Collecting objective and subjective data is necessary for accurate assessments. The RN develops a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate. Designing actions and interventions relevant to the patient’s culture and age is required. Determining desired outcomes in coordination with the patient and family according to the medical plan of care is essential. Patient care therapies and procedures are to be fully implemented within the scope of practice. Explaining procedures, treatments, and processes to patients and families is required. The RN initiates actions to alleviate patient stress and discomfort. Recognizing, responding to, and communicating changes in patient conditions is a critical aspect of the role. Conducting and documenting ongoing assessments in accordance with hospital policy and established standards is necessary. Delegating tasks appropriately and overseeing team members in the performance of activities regarding patient care is vital. Monitoring the accuracy and completeness of delegated patient care provided by other team members is essential. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is required. Facilitating the patient discharge process by executing discharge orders in a timely manner is necessary. Providing input into patient/nurse assignments considers available staffing, unit activity, and patient acuity. Active participation in unit-based activities that support organizational values and priorities is expected. Promoting a positive group dynamic and a healing environment through words and actions is essential. Assuring nursing excellence in accordance with accepted standards and evidence-based practice is critical. Participation in department and/or unit quality improvement activities and core measures is required. Using a holistic perspective in patient care is essential.
