Travel Nurse RN - Med Surg - $2,380 per week

GHR Healthcare is seeking a Travel Home Health Registered Nurse (RN) to serve Valparaiso, Indiana and surrounding counties of Northwest Indiana on a 13‑week contract. The position requires 8‑hour shifts, 40 hours per week, and involves comprehensive home‑care nursing responsibilities. Job Details Location: Valparaiso, IN (46383) – serving Northwest Indiana & surrounding counties Assignment Length: 13 weeks (April 26, 2026 – July 25, 2026) Shift: Monday – Friday, 8:00am – 4:30pm (8‑hour shifts, 40 hrs/week guaranteed 30–40 hrs) On‑Call/Weekend: Rotating weekend on‑call Travel: Mileage reimbursed between patient visits (valid driver’s license and reliable vehicle required) Dress Code: Black scrubs Weekly Estimated Pay: $2,120 – $2,230 Job Requirements / Qualifications Active Registered Nurse (RN) license in Indiana Associate’s degree in Nursing or higher (ADN or BSN preferred) Current Basic Life Support (BLS) certification Minimum 1 year of nursing experience; home health or homecare experience preferred Experience with Homecare Homebase (HCHB) and OASIS training is a plus Strong communication, critical thinking, and professionalism in home health settings Ability to work independently in patient homes and travel within assigned counties Primary Responsibilities Perform comprehensive home health assessments and ongoing monitoring of patient health status in the home setting Provide skilled nursing visits (wound care, medication management, IV therapy as applicable) and implement physician orders accurately Document care timely and accurately using electronic health record systems (EHR), including HCHB when applicable Coordinate interdisciplinary care with physicians, therapists, case managers, and family members to support patient‑centered plans Participate in weekly team meetings and case conferences to review patient progress and care plans Educate patients and caregivers on disease management, medications, wound care, and home safety Meet productivity expectations (25–30 points per week) while maintaining high‑quality clinical care and compliance with home health regulations Benefits 401K with Matching Healthcare, Dental and Vision Insurance Equal Opportunity We are an equal opportunity employer and value diversity across our organization.
