Home Hospice RN Quality Coordinator

This in-office Quality Coordinator position supports home health and hospice patients while being based in the vibrant community of Martinez, California. You will focus on reviewing clinical documentation, ensuring regulatory and policy compliance, and supporting clinicians who care for patients in their homes and in hospice settings. A typical day includes conducting chart audits, checking documentation for completeness and accuracy, monitoring adherence to clinical standards and regulatory requirements, and collaborating with nursing and interdisciplinary teams to address any gaps or opportunities for improvement. You will help maintain survey readiness and support performance improvement projects, using EMR systems and quality data to inform recommendations. The position involves close coordination with field staff to promote consistent, high-quality patient care. You will provide feedback on documentation practices, assist with education on policies and procedures, and support the implementation of quality initiatives that enhance patient outcomes and experiences. While patient ratios and bed counts vary by service line, your focus will be on ensuring that each patient's record reflects safe, appropriate, and compassionate care in accordance with home health and hospice standards. Schedules are typically structured around daytime office hours, with expectations set by the organization for availability to support chart review, team communication, and quality-related tasks.
