Work Environment

This position will be primarily onsite, working within a vibrant school setting in Flushing. As part of our healthcare team, you will engage with students, families, and staff, addressing health concerns in a supportive educational environment. Your work hours will typically be from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. Some weekend participation in community outreach events and health fairs may be required. Key Responsibilities

• Conduct comprehensive health assessments and screenings for students and their families, helping to identify medical and psychological needs.
• Diagnose and manage acute and chronic medical conditions, providing preventive care and health promotion tailored to individual patients.
• Collaborate with local organizations to develop and implement health education initiatives, focusing on chronic disease management, wellness, and preventive care.
• Advocate for patients’ rights, ensuring access to healthcare services and culturally sensitive care practices.
• Coordinate referrals and follow-up care, maintaining continuity of care to ensure optimal patient health outcomes.
• Maintain accurate and detailed medical records per regulatory standards, ensuring compliance with all health regulations.
• Engage in community outreach activities, representing Empower Health NP at health fairs and support clinics, where you can provide important health screenings and education.
• Stay informed on the latest community health trends and best practices by participating in ongoing professional development opportunities and peer review.

Patient Population / Client Focus

This role primarily focuses on the student population within the school setting, providing a unique blend of acute and preventative health services. You will work to support not only the physical well-being of children but also their mental and emotional health. Building trust with students and their families will be critical to ensuring they receive the appropriate care and education necessary for a healthier future.

Qualifications

• A Bachelor’s Degree in Nursing from an accredited program is required.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) is essential.
• DEA# and NPI# are required.
• Strong clinical assessment skills and the ability to develop individualized care plans are necessary.
• Excellent communication and interpersonal skills, with the ability to connect effectively with a diverse range of patients and families.
• Proficiency in using electronic health record (EHR) systems to manage patient data efficiently.
• A minimum of 3 years of relevant clinical experience, preferably in a community health or public health setting.

Preferred Experience

• Experience in primary care or family medicine, engaging with low-income populations, and addressing health disparities.
• A background in school-based health programs or outreach initiatives.
• A passion for working with children and families in a preventive care setting.
