Key Responsibilities

• Collaborate with regional operations and clinical leadership to meet organizational goals and improve patient care delivery.

• Provide an excellent customer experience to foster high levels of patient satisfaction and retention.
• Adhere to the Scope and Standards of Practice, ensuring ethical practice in line with your board certification and relevant regulations.
• Work alongside primary care providers and interdisciplinary teams to coordinate comprehensive care plans for patients.
• Manage acute and chronic conditions through assessment, diagnosis, and adjustments of treatment plans based on lab and diagnostic interpretations.
• Efficiently complete all required regulatory visits and assume responsibility for follow-ups as needed.
• Organize and participate in family conferences, offering advanced care planning and education.
• Implement pharmacy recommendations and maintain open communications with pharmacists regarding patient medications.
• Maintain accurate and compliant documentation in our electronic medical record (EMR) system.
• Monitor patient progress during assigned hours in facilities, ensuring adequate face-to-face coverage.
• Provide comprehensive health education to empower patients about their health conditions and treatment strategies.

Patient Population / Client Focus

As a Nurse Practitioner at Expert Cares Healthcare Services, you will primarily serve elderly patients, including those in assisted living, independent living, and skilled nursing facilities. A strong emphasis will be placed on addressing the unique healthcare needs of low-income populations, requiring not only medical expertise but also cultural sensitivity and a compassionate approach to care.

Qualifications

• Must possess U.S. work authorization.
• A Bachelor’s Degree in Nursing, followed by a Master of Science in Nursing, Doctor of Nursing Practice, or a graduate of an NP Certificate Program.
• Active and unrestricted state license to practice as a Nurse Practitioner.
• Current national certification from AANP or ANCC.
• An active DEA license is required for prescribing medications.
