Work Environment

This full-time position is based onsite at our state-of-the-art hospital located in Mount Vernon, New York. Our facility is equipped with advanced medical technology and resources, enabling our healthcare providers to deliver high-quality care. You will be part of a multidisciplinary team, collaborating closely with physicians, nurses, and specialists to ensure comprehensive and seamless patient care. Your typical working hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and community outreach events, totaling approximately 40 hours a week.

Key Responsibilities

• Perform comprehensive health assessments and screenings, utilizing evidence-based practices to evaluate the physical, psychological, and social health of patients.
• Diagnose and manage a variety of acute and chronic medical conditions, providing preventive care and health education to promote optimal patient health outcomes.
• Develop individualized care plans and treatment strategies in collaboration with patients and their families, addressing specific health needs and goals.
• Engage with community organizations and stakeholders to design and implement health education initiatives aimed at improving knowledge and preventive health practices within the population.
• Ensure continuity of care by coordinating referrals and follow-up procedures, maintaining a thorough communication channel with all healthcare providers involved in a patient’s care.
• Maintain accurate and confidential medical records in compliance with HIPAA and other regulatory requirements, utilizing Electronic Health Records (EHR) systems.
• Participate in community outreach activities, health fairs, and other engagements to enhance public awareness of health issues and available services.
• Stay informed about the latest developments in community health, evidence-based practices, and healthcare policies, integrating this knowledge into clinical practice.

Patient Population / Client Focus

You will be responsible for providing care to a diverse patient population, including individuals from various backgrounds, age groups, and health conditions. Your ability to deliver culturally sensitive care will be essential in fostering trusting relationships and improving patient satisfaction. Through effective communication and advocacy, you will help to empower patients in managing their own health and navigating the healthcare system.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by recognized credentialing bodies.
• Current and unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a valid DEA# and NPI#.
• Minimum of three years of clinical experience as a Nurse Practitioner.
• Excellent clinical assessment, diagnostic skills, and a deep commitment to patient-centered care.
• Strong communication and interpersonal skills, with an ability to work both independently and collaboratively.
• Proficiency in using EHR systems effectively to document patient care and coordinate treatment plans.

Preferred Experience

• Previous experience in primary care or family medicine is highly preferred and will enhance your ability to meet the demands of this role.
• Experience working in community health or public health settings is also a plus, as it aligns with our organizational goals of community engagement and health education.

Compensation and Benefits

The annual salary for this position ranges from USD 150,000 to 160,000, commensurate with experience and qualifications. Additionally, you may be eligible for a sign-on bonus of up to $10,000.
