Work Environment

This position is based onsite at a local school facility in Mount Vernon, a vibrant city known for its diverse population and community-centric values. Work hours will primarily be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community health events or outreach initiatives during weekends may also be required. Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, focusing on identifying both immediate health needs and long-term wellness goals.
• Diagnose and create treatment plans for managing acute and chronic medical conditions while promoting preventive health practices through education.
• Collaborate closely with educators, parents, and other healthcare providers to implement effective health education programs tailored to the school population.
• Advocate for equitable access to healthcare services by providing culturally sensitive care to ensure all families are supported.
• Ensure continuity of care through proper referrals and follow-up management while utilizing electronic health record (EHR) systems to maintain accurate patient documentation.
• Engage in community outreach events to disseminate health information, conduct screenings, and heighten awareness of available resources.
• Stay informed on current community health trends and develop best practice protocols to enhance service delivery.

Patient Population / Client Focus

The primary focus of this role is on school-aged children and their families. You will provide preventive care, assess and treat various health conditions, and promote health education, ensuring that every individual receives personalized attention. Your ability to connect with young patients and their families will be crucial in fostering trust and encouraging adherence to health recommendations.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution is required.
• Current and unrestricted Nurse Practitioner (APRN) license in New York State is mandatory.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the ANCC or AANP.
• Possession of a valid DEA# and NPI# is required for practice.
• Demonstrated strength in clinical assessment and diagnostic skills with a patient-centered approach.
• Strong interpersonal and communication skills to interact effectively with a diverse population.
• A proactive and collaborative mindset with the ability to work independently as well as a part of a multidisciplinary team.

Preferred Experience

• A minimum of 2 years of experience as a Nurse Practitioner, preferably in primary care, family medicine, or community health settings.
• Experience utilizing electronic health records (EHR) systems effectively in managing patient information.
• Familiarity with public health initiatives and community outreach methodologies is highly valued.

Compensation and Benefits

This role offers a competitive salary range between $150,000 and $160,000 per year, commensurate with experience and qualifications. Additional benefits include a sign-on bonus of up to $10,000, health insurance, retirement plans, and continuing education allowances.
