Key Responsibilities

• Design, implement, and monitor individualized treatment plans tailored to the specific needs of residents, communicating effectively with patients, families, and other healthcare providers.
• Order and interpret lab tests and diagnostic studies, utilizing your clinical expertise to inform adjustments to treatment as necessary.
• Maintain accurate and compliant patient records within our electronic medical record (EMR) system, ensuring all documentation adheres to state and federal regulations.
• Engage in regular collaboration with clinical leadership to align operational goals and improve care quality.
• Participate in strategic planning regarding value-based care models to enhance patient outcomes.
• Provide education and resources to residents and families, empowering them to manage their health effectively.
• Participate actively in family conferences and advanced care planning discussions.
• Conduct chronic and acute assessments, addressing any clinical issues and collaborating with specialists as necessary.
• Complete all required regulatory visits, maintaining a proactive approach to resident health management.
• Ensure adequate face-to-face coverage by being present in the facility during work hours.

Qualifications

• An active, unrestricted Nurse Practitioner license in the state of New York.
• Current DEA license for medication prescribing.
• Proof of U.S. work authorization.
• A commitment to providing healthcare to senior and low-income populations.

Preferred Experience

• Experience in geriatric care, preferably in assisted living or skilled nursing facilities.
• Demonstrated ability to work independently while also being part of a healthcare team.
• Familiarity with EMR systems and the use of telemedicine tools.

Compensation and Benefits
