This role is situated onsite in an Assisted Living Facility, where teamwork and collaboration are essential. You will be working closely with physicians, nursing staff, and other healthcare professionals to create a holistic approach to care. Key Responsibilities

• Order, perform, and interpret diagnostic tests, including complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (X-rays) to assess and monitor patient health.
• Thoroughly analyze patient history, symptoms, and diagnostic information to develop accurate diagnoses and recommend appropriate treatment options.
• Develop and implement comprehensive treatment plans based on current care standards and professional guidelines to ensure optimal management of health conditions.
• Diagnose and treat a variety of acute health issues, such as infections and injuries, while coordinating referrals to specialists when necessary.
• Prescribe medications by considering factors such as efficacy, safety, cost, and individual patient needs, ensuring all prescriptions comply with legal requirements.
• Recommend lifestyle and behavioral interventions to address health risks, fostering a proactive approach to wellness among residents.
• Educate patients and their families about managing acute and chronic conditions, tailoring instructions to suit their specific situations.
• Utilize Electronic Health Records (EHR) proficiently, ensuring accurate and timely documentation of patient information in alignment with facility policies.
• Evaluate patients' medical and psychological conditions in a holistic manner, focusing on improving overall wellbeing.

Patient Population / Client Focus

This position focuses on providing healthcare to adults and seniors, encompassing a diverse range of medical needs. As a Nurse Practitioner in an Assisted Living Facility, you will encounter patients with varying degrees of chronic illnesses, functional limitations, and mental health challenges. Qualifications

• Current Nurse Practitioner certification issued by the New York State Education Department (NYSED).
• A valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA), which can be obtained through facility orientation.
• A valid New York State license and current registration to practice as a Registered Professional Nurse (RN) from the NYSED.

Preferred Experience

Experience working in geriatric or assisted living settings is preferred but not required. Familiarity with chronic disease management and the challenges associated with elder care will be advantageous. Strong interpersonal skills and the ability to build rapport with patients and their families are essential to effectively carry out responsibilities.

• Annual salary range of USD 150,000 – 160,000, commensurate with experience and qualifications.
