Work Environment

This role is primarily onsite at our hospital in Mount Vernon, New York. You will work in a dynamic, team-oriented environment alongside physicians, specialists, nursing staff, and caregivers. The hospital provides a diverse range of healthcare services, making it an ideal setting for practicing advanced nursing care and engaging in meaningful patient interactions. Key Responsibilities

• Develop individualized treatment plans based on comprehensive assessments of patients' physical and emotional health, effectively communicating these plans to residents, caregivers, and other healthcare providers.
• Order and interpret laboratory tests and diagnostic procedures, adjusting treatment plans in response to findings and patient needs.
• Maintain accurate and compliant electronic medical records (EMR), ensuring timely documentation of patient interactions and care plans.
• Collaborate with local and regional operational and clinical teams to improve patient care practices and meet organizational goals.
• Support strategic initiatives focused on value-based care, contributing to the overall success of our healthcare programs.
• Provide a high level of customer service, ensuring exceptional patient experiences that enhance satisfaction and retention rates.
• Practice in accordance with ethical standards, relevant state and federal regulations, and the Scope and Standards of Practice for nurse practitioners.
• Engage with primary care providers and interdisciplinary teams to coordinate comprehensive, patient-centered care.
• Participate actively in regulatory visits, family conferences, and advanced care planning discussions to guide treatment decisions.
• Assess, diagnose, and manage chronic and acute health conditions, offering education and support to patients and their families.

Patient Population / Client Focus

The primary focus of this role is on senior patients, particularly in skilled nursing and assisted living settings. You will be responsible for providing high-quality, compassionate care to individuals facing various health challenges, including chronic illnesses, mobility issues, and cognitive impairments. Your efforts will contribute significantly to enhancing their quality of life and maintaining their independence as much as possible.

Qualifications

• A Bachelor’s Degree in Nursing and a completed nurse practitioner graduate program are required.
• National certification through AANP or ANCC.
• An active, unrestricted nurse practitioner license in New York State.
• An active DEA license.
• Ability to work with diverse populations, demonstrating a passion for serving low-income and senior individuals.
• US work authorization.

Preferred Experience

• Master of Science in Nursing or Doctor of Nursing Practice degree.
• Prior experience providing healthcare in senior living facilities, hospitals, or similar environments.
• Familiarity with electronic medical records (EMR) systems and telemedicine platforms.
• Experience in value-based care models and an understanding of regulatory compliance in healthcare.

Compensation and Benefits

We offer a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes:
