Key Responsibilities

As an Internal Medicine Nurse Practitioner, you will:

• Deliver high-quality primary care to a diverse patient population, managing both routine and acute medical issues.
• Conduct thorough office visits, performing comprehensive assessments and crafting detailed care plans.
• Administer annual wellness visits and health risk assessments, emphasizing accurate documentation and holistic approaches to health.
• Facilitate care coordination with a wide range of healthcare providers, including specialists, testing facilities, and community agencies.
• Lead initiatives in population health management, ensuring compliance with evidence-based care guidelines.
• Participate in phone triage and outreach to follow up with patients, offering educational resources regarding their health conditions.
• Engage in community health promotional activities that foster wellness and disease prevention.
• Conduct home visits to ensure continuity of care for those unable to attend clinic appointments.
• Report directly to the Medical Director and collaborate closely with operational leadership to enhance clinical practices.

Patient Population / Client Focus

Your primary focus will be on older adults receiving Medicare benefits, necessitating an empathetic approach to their unique healthcare challenges. By leveraging your expertise in Internal Medicine, you will be integral in helping these patients maintain their health, manage chronic conditions, and enhance their overall well-being. Qualifications

The ideal candidate will possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized bodies such as AANP or ANCC.
• An active and non-probationary Nurse Practitioner license in the state of New York.
• A current and valid DEA license.
• Previous experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Demonstrated understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Compensation and Benefits

This full-time role offers a competitive salary range of $115,000 to $150,000 per year, commensurate with experience and expertise. Additional benefits include:
