Work Environment

This position is onsite, primarily focused on home care. Operating in a supportive and collaborative environment, you will work with a multidisciplinary team dedicated to improving community health. Our office hours are typically from 9:00 a.m. to 6:00 p.m., with some flexibility for evening clinics until 8:00 p.m. You may also participate in community outreach events on weekends to further our mission of health equity.

Key Responsibilities

• Conduct comprehensive health assessments, ensuring thorough evaluations that include history taking, physical exams, and diagnostic testing as necessary.
• Diagnose and manage acute and chronic medical conditions, providing evidence-based treatment tailored to individual patient needs.
• Develop and implement personalized care plans that promote health literacy and empower patients to engage in their health journey.
• Collaborate with community organizations to design and deliver health education programs focused on prevention and wellness strategies.
• Advocate for patients' rights and needs, ensuring culturally competent care across diverse populations.
• Coordinate ongoing patient care, including follow-ups, chronic disease management, and referrals to specialists as appropriate.
• Maintain accurate and compliant medical records in accordance with legal and organizational standards to ensure quality patient care.
• Regularly participate in health fairs and community outreach initiatives to promote health awareness and access to services.
• Stay informed about the latest advancements in internal medicine, evidence-based practices, and relevant public health initiatives.

Patient Population / Client Focus

You will primarily serve a diverse patient population in the Bronx, focusing on individuals and families who often face barriers in accessing healthcare. This role emphasizes the importance of outreach, culturally sensitive care, and tailored wellness programs to address the specific needs of our community members.

Qualifications

• Current certification as a Nurse Practitioner and active nursing license in the state of New York.
• A Master's degree in Nursing from an accredited institution.
• Strong interpersonal skills, with the ability to work collaboratively within a multidisciplinary team.
• Exceptional communication skills, enabling effective interaction with patients from diverse backgrounds.
• A commitment to promoting health equity and advocating for marginalized communities.

Preferred Experience

• A minimum of three (3) years of clinical experience, preferably in community health settings.

Compensation and Benefits

The competitive salary range for this position is between USD 120,000 and 130,000 annually, dependent on your experience and qualifications.
