Work Environment

This position is onsite, involving direct interactions with patients in their homes. Key Responsibilities

As an Internal Medicine NP/PA, you will undertake various responsibilities that are crucial for patient health management, including:

• Conducting comprehensive patient assessments through interviews and physical examinations to develop a clear understanding of their health status.
• Obtaining, updating, and analyzing medical histories to inform present and future care decisions.
• Developing detailed management plans for each patient, focusing on both immediate needs and long-term health goals.
• Collaborating with physicians to address complex health issues and facilitate timely interventions.
• Educating patients and their families about health management, preventive care, and disease management strategies.
• Documenting all interactions and maintain accurate medical records in compliance with regulatory standards and organizational protocols.

Patient Population / Client Focus

You will primarily serve patients with diverse internal medicine needs, addressing various chronic and acute health conditions. The role emphasizes building strong relationships with patients, many of whom may be dealing with multiple health challenges, ensuring a personalized approach that prioritizes their comfort, choice, and overall well-being.

Qualifications

• Must possess a Bachelor’s Degree in nursing or a related field with Nurse Practitioner or Physician Assistant credentials.
• Must hold a current NP/PA license in New York State.
• Completion of a graduate program from an accredited medical or nursing institution.
• Successful completion of the Physician Assistant National Certifying Examination is preferred.
• Ability to demonstrate strong clinical assessment skills, effective communication, and compassionate care.

Preferred Experience

We prefer candidates with a minimum of two years’ experience in an ambulatory care setting. Familiarity with home care practices, electronic health record systems, and chronic disease management will be highly advantageous. Experience in internal medicine or related specialties will also be considered a plus.

Compensation and Benefits

This position offers a competitive salary in the range of USD 150,000 – 160,000 annually, commensurate with experience and qualifications. Additionally, we provide a comprehensive benefits package, which includes:
