Key Responsibilities

• Collaborate effectively with physicians and multidisciplinary teams to deliver comprehensive healthcare services from preventive measures to acute care for the assigned patient population.
• Conduct thorough assessments by acquiring complete medical, physical, and psychosocial history, followed by comprehensive health evaluations tailored to the patient's age and history.
• Perform an array of non-invasive preventive screening procedures based on established clinical guidelines and tailored to individual patient needs.
• Develop differential diagnoses informed by patient history, physical examinations, and relevant clinical findings to identify the right course of action for each patient.
• Create, implement, and adjust relevant patient education plans to empower patients in managing their health conditions effectively.
• Prescribe necessary pharmacologic and non-pharmacologic treatments to achieve optimal patient health outcomes while adhering to established protocols.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient base, ranging from young adults to seniors, with an emphasis on managing chronic diseases, acute conditions, and preventive healthcare. Your expertise will directly impact patient outcomes by ensuring personalized and effective care strategies that address individual health challenges.

Qualifications

• A Master's Degree in Nursing with a specialization as a Nurse Practitioner.
• An active Registered Nurse (RN) license in the state of New York, ensuring compliance with state regulations and practice standards.
• At least 3 to 4 years of relevant clinical experience, demonstrating your competence in managing a variety of medical conditions.
• Proficiency in using medical technology, including electronic health records (EHR) systems, to document and manage patient information securely.
• Strong communication skills, particularly in patient education and advocacy, enhancing the patient experience and care outcomes.
• Experienced in diagnosing and treating a broad spectrum of medical conditions, showcasing your capacity to develop and implement effective treatment plans.

Preferred Experience

• Experience in an internal medicine or multi-disciplinary care setting is preferred, as it will enhance your ability to contribute effectively in this role.
