Key Responsibilities

• Perform comprehensive assessments of patient health through detailed history-taking and thorough physical examinations.
• Develop and implement individualized management plans for patients with acute and chronic internal medicine conditions.
• Facilitate continuity of care by regularly reviewing and updating patient care plans and maintaining accurate medical records.
• Collaborate closely with attending physicians to address short-term and long-term health concerns, ensuring that each patient receives comprehensive care tailored to their unique needs.
• Educate patients and their families about health management, disease prevention strategies, and lifestyle modifications.
• Participate in quality improvement initiatives aimed at enhancing patient services and operational efficiencies.
• Remain up-to-date with current best practices and engage in continuous professional development activities.

Patient Population / Client Focus

You will primarily serve a diverse patient population that includes school-aged children and young adults. This role is vital in addressing their internal medicine needs, focusing on preventive care, chronic disease management, and health education. Qualifications

• A Bachelor’s Degree in Nursing or a related field from an accredited institution.
• A current and valid NP or PA license in New York State.
• Successful completion of the Physician Assistant National Certifying Examination (PANCE) or evidential Nurse Practitioner certification.
• At least two years of experience in an ambulatory care setting is preferred, enhancing your ability to manage a wide range of patient conditions.

Preferred Experience

Compensation and Benefits
