Key Responsibilities

• Conduct thorough initial patient histories and physical examinations to identify health issues and develop appropriate treatment plans.
• Initiate, document, and communicate individualized care plans based on comprehensive assessments, ensuring coordination among multidisciplinary teams.
• Perform daily patient rounds, monitoring progress, and adjusting treatment protocols as necessary to optimize patient care.
• Write and manage orders for medications, laboratory tests, and diagnostic imaging, ensuring compliance with clinical guidelines.
• Interpret laboratory and diagnostic test results, making informed decisions to enhance patient care outcomes.
• Collaborate with attending physicians, residents, nursing staff, and other care providers to ensure that all aspects of patient care are addressed effectively.
• Document discharge summaries, prescriptions, and referrals accurately to maintain comprehensive patient records.
• Provide clear post-discharge instructions and support to patients and their families, facilitating a smooth transition from hospital to home care.
• Engage in performance improvement initiatives, clinical research, and ongoing education to promote professional development and organizational excellence.

Patient Population / Client Focus

Your role primarily involves caring for adults with diverse health needs, ranging from chronic conditions to acute medical issues. You will be instrumental in creating patient-centered care plans that emphasize health education, chronic disease management, and preventative care, tailoring your approach to meet the unique needs of each patient.

Qualifications

• A Master's Degree in a Nurse Practitioner Program is required, along with National Board Certification in your specialty area.
• A valid and current license to practice as a Nurse Practitioner in New York State and as a Registered Professional Nurse is essential.
• Possession of a collaborative practice agreement with designated protocols filed with the NYS Department of Education, or a collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.
• Current Basic Life Support (BLS) certification is mandatory for patient safety and emergency response.

Preferred Experience

While not required, prior experience in home care, internal medicine, or gerontology will be considered a strong advantage. Familiarity with electronic health records (EHR) and proficiency in telehealth services are also beneficial in enhancing patient care coordination.

Compensation and Benefits

This position offers a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and expertise.
