Work Environment

Working onsite, you will be part of a dynamic and multidisciplinary care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and support staff such as dedicated medical scribes and care managers. This collaborative setting allows for efficient care coordination and communication, optimizing patient engagement and care quality. Key Responsibilities

• Deliver comprehensive primary care to adult patients, including those with complex health needs.
• Lead office visits to address routine and acute medical issues, providing prompt and effective treatment.
• Conduct annual wellness visits and health risk assessments, focusing on holistic evaluations and maintaining meticulous documentation standards.
• Coordinate care with specialists and other healthcare providers to ensure comprehensive management of patients’ health conditions.
• Champion population health initiatives by tracking preventive measures, such as facilitating breast cancer screening for eligible female patients.
• Assist the care team with phone triage and outreach to ensure that patient needs are met efficiently.
• Educate patients about their health conditions, treatment options, and self-management strategies to promote informed decision-making.
• Engage in community outreach and promotional activities to enhance program visibility and patient awareness.
• Conduct home visits as required, further personalizing care based on individual patient circumstances.
• Report directly to the Medical Director and collaborate closely with operational leadership to support the organization's objectives.

Patient Population / Client Focus

This role primarily focuses on serving older adults on Medicare, ensuring they receive comprehensive and personalized care that aligns with their unique health profiles. By maintaining a strong emphasis on building patient relationships, our Nurse Practitioners enhance patient trust and engagement, which are critical components in fostering successful health outcomes.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Nurse Practitioner in Family, Adult, or Adult-Gerontology Primary Care through AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Current DEA license to prescribe medications.
• Previous experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Proficient in HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and conducting health risk assessments.
• A true passion for teamwork, collaboration, and contributing to a patient-centered focus.

Preferred Experience

While not required, candidates with experience in home care settings or those familiar with community health initiatives will be given preference. Experience in utilizing electronic health record (EHR) systems to document patient care and outcomes is also highly desirable.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to $160,000 annually, commensurate with experience and qualifications.
