Work Environment

This position is primarily onsite, allowing for direct interaction with both patients and the multi-disciplinary care team. You will be part of a collaborative environment that includes physicians, physician assistants, medical assistants, and a dedicated medical scribe. Additionally, our team receives support from registered nurses and care managers, allowing you to focus on providing exceptional patient care within our outpatient practice, while also partnering with a network of elite specialists and hospitals for any necessary specialty and acute care.

Key Responsibilities

• Deliver individualized primary care, focusing on prevention, wellness, and chronic disease management.
• Conduct thorough office visits to address both routine and acute health concerns, adapting care strategies to meet the needs of each patient.
• Administer annual wellness visits and comprehensive health risk assessments, emphasizing accurate documentation to evaluate patient health holistically.
• Coordinate care with other healthcare providers, specialists, and testing facilities, ensuring streamlined communication and continuity of care for patients.
• Lead population health initiatives, ensuring patients receive evidence-based screenings and preventive care measures.
• Assist the care team by providing phone triage support and outreach to enhance patient engagement and follow-up.
• Educate patients and their families about health conditions, treatment plans, and preventive measures, empowering them to become active participants in their care.
• Participate in community promotional activities to raise awareness about our services and enhance community health outcomes.
• Conduct home visits for patients requiring more personalized care in their own environments.
• Report to the Medical Director, collaborating closely with operational leadership to meet organizational goals.

Patient Population / Client Focus

Our practice predominantly serves adults on Medicare, many of whom require specialized attention due to chronic conditions or complex health needs. By providing personalized primary care, we strive to help our patients lead fulfilling lives, maintaining their dignity and autonomy while effectively managing their health. Your expertise as an Adult-Gerontology Nurse Practitioner will be pivotal in nurturing these relationships and improving patient outcomes.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing medications.
• A background in primary care, specifically within internal medicine, geriatrics, or family medicine.
• Knowledge in HCC documentation, ICD-10 coding, and health risk assessments, ensuring compliance with regulatory standards.
• A strong commitment to teamwork, with a willingness to collaborate across disciplines to enhance patient care.

Preferred Experience

• Experience working within a Concierge Medicine or House Calls model is highly desirable.
• Previous leadership experience in population health initiatives or quality improvement projects can be advantageous.
• Demonstrated success in building patient rapport and delivering exceptional care in a fast-paced environment.

Compensation and Benefits

The position offers a competitive annual salary ranging from USD 120,000 to 160,000, commensurate with experience and qualifications.
