The Jewish Community Health Center in Yonkers, New York, is actively seeking a dedicated and skilled full-time Nurse Practitioner (NP) to join our team within the Center for Advanced Cardiac Care (CACC). This role is ideal for individuals passionate about providing high-quality cardiovascular care and improving patient health outcomes. Under the guidance of the Chief Nurse Practitioner and the Medical Director, the NP will play a crucial role in outpatient cardiac services, focusing on preventing and managing cardiovascular diseases.

Work Environment

This position is located onsite within a school facility that serves as a community health hub, which enhances our access to local patients and families. Working Monday through Friday from 9 AM to 5 PM, this role does not require evening or weekend shifts, allowing for a healthy work-life balance. The collaborative atmosphere encourages open communication and integration among healthcare professionals, including physicians, nurse practitioners, and registered nurses.

Key Responsibilities

• Collaborate closely with cardiologists and other healthcare professionals to provide comprehensive care to patients.
• Evaluate and manage patients diagnosed with hypertension, high cholesterol, and other cardiovascular risk factors.
• Provide specialized outpatient Electrophysiology services for patients experiencing abnormal electrocardiograms and arrhythmias, including follow-up care for device patients.
• Address acute conditions such as Coronary Artery Disease, Peripheral Artery Disease, Congestive Heart Failure, valvular heart disease, and stroke/transient ischemic attack.
• Conduct stress testing when necessary to assess cardiac function under physical exertion.
• Perform comprehensive patient histories and physical examinations, prescribing medications as needed, and referring patients to specialists when appropriate.
• Educate patients and their families on health management strategies, medication adherence, and lifestyle modifications to promote cardiovascular health.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.

Patient Population / Client Focus

The Nurse Practitioner will predominantly interact with patients seeking care for various cardiac conditions within the outpatient setting. Our clientele includes individuals with chronic cardiovascular issues as well as those presenting acute symptoms. The role emphasizes education and ongoing support to improve the overall cardiac health of the patient population while addressing social determinants of health affecting their wellbeing.

Qualifications

• Completion of an accredited Nurse Practitioner program with a focus relevant to cardiology.
• A Master’s degree or doctoral degree is required.
• Current licensure to practice as a Nurse Practitioner in New York State.
• Certification in a relevant specialty area is preferred.

Preferred Experience

• Prior experience in cardiology, either in an inpatient or outpatient setting, is highly valued.
• Experience working collaboratively within a multidisciplinary healthcare team.
• Familiarity with electronic medical records (EMR) systems and documentation practices.

Compensation and Benefits

The position offers a competitive salary ranging from USD 120,000 to 160,000 per year, commensurate with experience and qualifications.
