Work Environment

This position is onsite, providing home care services in the Yonkers area. As a Nurse Practitioner, your work environment will be diverse, moving between patient homes, community health centers, and local outreach events. Your scheduled hours will generally span from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. Weekend engagement in community health fairs or outreach activities may also be required. An average workweek will total around 40 hours, allowing you to manage your time effectively while fulfilling your responsibilities.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients in home settings, accurately documenting health histories and concerns.
• Diagnose and manage both acute and chronic medical conditions, enabling effective treatment plans focused on preventive care and health promotion.
• Collaborate with local community organizations to design and implement health education initiatives that empower individuals to take charge of their health.
• Provide culturally competent care, ensuring patients' rights are respected and their specific needs met.
• Coordinate referrals and follow-up care, working closely with specialists to guarantee seamless continuity of care.
• Maintain comprehensive and precise medical records in line with healthcare regulations, ensuring confidentiality and regulatory compliance.

Qualifications

• Current certification as a Nurse Practitioner with a valid, active state nursing license.

Compensation and Benefits
