Work Environment

This full-time position is primarily onsite, focusing on home care within the community of Mamaroneck. Our typical work-hours are from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Engaging in community outreach events and health fairs, particularly on weekends, is an integral part of this role, helping to promote preventative care and education.

Key Responsibilities

• Perform thorough health assessments and screenings for individuals and families in diverse settings.
• Diagnose and manage a range of acute and chronic medical conditions while promoting preventive health strategies.
• Work collaboratively with community organizations to design and implement impactful health education programs.
• Provide culturally competent care, advocating for the rights of patients and facilitating access to various healthcare services.
• Coordinate referrals and follow-up care with local healthcare providers to ensure continuity and efficacy in patient care.
• Maintain and update comprehensive medical records in accordance with regulatory standards and best practices.
• Actively participate in community outreach, providing health screenings and educational seminars to foster community engagement.

Qualifications

• Valid Nurse Practitioner certification recognized by the nursing board.

Compensation and Benefits

The Nurse Practitioner position offers a competitive annual salary range of $180,000 to $200,000, commensurate with experience and qualifications.
