Work Environment

This full-time position is primarily located onsite at our outpatient office in New Rochelle, known for its friendly and collaborative atmosphere. As a Nurse Practitioner, you will work alongside a multidisciplinary team of healthcare professionals dedicated to transforming community health outcomes. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional extended hours during evening clinics and participation in weekend health fairs or outreach events.

Key Responsibilities

• Conduct thorough health assessments, including physical examinations, patient histories, and diagnostic testing to identify both acute and chronic health issues.
• Diagnose and effectively manage a variety of medical conditions, providing evidence-based preventive care and facilitating health promotion initiatives.
• Collaborate with local community organizations and public health stakeholders to design and execute health education programs aimed at increasing awareness of prevalent health issues.
• Deliver culturally competent care while advocating for patients’ rights, ensuring that all individuals receive fair access to healthcare services.
• Coordinate patient referrals and manage follow-up care to establish and maintain effective continuity of care, ensuring that all patients receive the resources they require.
• Maintain meticulous and compliant medical records, utilizing electronic health record systems to document patient interactions accurately and efficiently.
• Participate in various community outreach programs and health fairs, performing health screenings and educating the public on various health topics.
