Work Environment

You will work onsite at our school-based clinic in the Bronx, where your full-time schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in weekend community outreach events or health fairs may be required, emphasizing our organization’s dedication to community involvement. This role averages approximately 40 hours per week, supporting a work-life balance while making a significant impact in the community.

Key Responsibilities

• Conduct thorough health assessments for patients, including physical examinations, screenings, and obtaining medical histories.
• Diagnose and manage both acute and chronic medical conditions, ensuring a focus on preventive care and health promotion strategies.
• Collaborate with local organizations to design and implement culturally responsive health education programs tailored to community needs.
• Advocate for patients’ rights, ensuring they have equitable access to necessary healthcare services and resources.
• Coordinate referrals and follow-up care, maintaining continuous communication to ensure patients receive appropriate treatments.
• Maintain accurate and compliant medical records, adhering to regulations and ensuring patient confidentiality.

Patient Population / Client Focus

This role predominantly focuses on serving the diverse student and family populations within the Bronx community. Qualifications

• Must hold current certification as a Nurse Practitioner and possess an active state nursing license.
• A Master’s degree in nursing from an accredited institution is required.

Compensation and Benefits
